Support plans and keywork
Shortly after you join one of our services, your support officer will work with you to assess your needs and agree a Support Plan. The plan will set out your individual needs and aspirations as well as how we plan to work with you to meet them. The goals and target dates you set as part of your plan are individual to you and can range from eating a healthy diet to paying your bills on time.
Your support officer will then meet with you on a regular basis to discuss progress towards your goals and to update your plan with any changes to your needs and goals – these meetings are called keywork. Your support officer will work with you to review your needs and goals at least every six months. If however you would like to review them earlier than this please ask your support officer.
We work with a range of partner agencies to provide additional specialist advice and support for our customers and will refer you to other services where appropriate.
In order to get the most out of your time with us, it is important that you commit yourself to the support process. When you join one of our services, your support officer will work with to agree and complete a support agreement which sets out what your rights and responsibilities are as well as what you can expect from our service.
If you no longer require one of our services, we will agree together when and how to end your support. If you live in one of our supported housing schemes we will work with you to find alternative accommodation.
